Add high contrast toggle button to dashboard, login, signup, admin,
report, setup, and 404 page navbars. Reorder nav items so contrast
and theme toggles appear before user info and auth links.

fix: use CSS class instead of inline style for contrast button active state ... d3cc7fe59f

Fixes specificity issue where inline style overrode hover state.
Also updates spec to remove maintenance.html (no navbar).
